Model I:

5 Session Face-to-Face EAP Assessment and Treatment
1 - 5 Sessions: $0 copayment
Rate Range: $2.20 - $3.20 per employee/per month

Model II:

10 Session Face-to-Face EAP Assessment and Treatment
1 - 5 Sessions: $0 copayment
6 - 10 Sessions: $15 copayment
Rate Range: 3.10 - $4.15 per employee/per month


Additional training can be provided for $150.00 per hour plus travel expenses.
* We use Language Line, if interpreter services are needed this is billed on a fee for service basis.

Managed Behavioral Health (Carve-out) Sample Design

A. Employee Assistance Program:

1, 3 or 5 Free Face-to-Face Counseling Sessions per Year

B. Outpatient Sessions (Contracted Providers):

C. Alternative Inpatient Care, Day Treatment, Partial Hospitalization, and Intensive Outpatient (Contracted Providers):

E. Inpatient Hospital (Contracted Providers):

5%, 10% or 15% Copayment


F. Non-Contracted Providers - Inpatient Hospital:

No Coverage to 30% Copayment for Emergency Hospitalization Only


G. Transitional Benefit:

Paid at $25.00 per Session for 20 Sessions per Initial Contract Year, per Family Unit or Existing Benefit


H. Emergency Ambulance Services:

10 - 20% Copayment

The Mental Health Inpatient Benefits Include:

14 Days - Unlimited Acute Care Hospital Days per Enrollee/per Year
2 Residential Days - 1 Hospital Day
2 Day Treatment Days = 1 Hospital Day
